Insurance Reminder

Employees must notify the Company by adding a Newborn child to coverage within

90 days of the birth. Coverage will be retroactive to the date of birth.

 

Part of your annual exam, Preventive Cancer Screenings ( such as Colonoscopy )

based on age and family medical history, does not require a $100.00 co-pay.

If you have a colonoscopy due to a medical diagnosis, you will be responsible

for the Outpatient Surgical Facility co-pay of $100.00. ( Limited to 2 Co-pays a year )

( Example : If you have medical complications, such as bleeding or pain. )

Out Patient Radiology co-pays are $100.00 for MRI, CAT, and PET imaging

( limited to 2 co-pays per year )

 

Our Prescription drug coverage is for a 21-day supply at your local pharmacy.

If your doctor writes a prescription for a 30 day supply, you will be charged a

double Co-pay.  Some drugs require precertification.  If your doctor doesn't

do this, you will pay the full cost of the drug. If there is a generic drug for a

name brand and the doctor writes, " Dispense as written " on the prescription, you

will be charged the full amount, unless there is a medical diagnosis that you can't

take the generic .  The drug has to be pre-approved before it is ordered to qualify

for the Co-pay.


Medical reminder

The colonoscopy procedure qualifies for short term disability for the

 day the procedure is performed.

Additional days are covered if complications arise from the

 procedure and if medically necessary.


Dental reminder

Replacement of teeth, you or a dependent lost prior to your

participation in the dental plan, are NOT COVERED.


Important

Although you may have shown your Insurance card to

 the hospital when you visited the emergency room,

the insurance information may not be shared with other areas.

If you receive a bill from an ambulance

service, a lab, a doctor's group that reads x-rays,

or the emergency room doctor's group; call them

right away with your insurance information , so they can bill your

 insurance carrier. Don't assume they already have it.

Waiting to contact them with the right information, might cause you to

be turned over to a collection agency.
